Personal Data Request Notice
Personal Data Request Notice pursuant to Utah Code 63A-19-402.
The Department of Health and Human Services (DHHS), Office of Recovery Services (ORS) requests or collects an individual’s personal data for the following reasons:
- To establish paternity; or,
- To establish or modify child support obligations; and,
- To recover Medicaid expenses.
ORS requests and collects personal data for the purposes of enforcing the established child support obligations or recovery of Medicaid expenses.
Personal data may also be used to locate individuals in order to carry out the stated purposes.
Personal data gathered by ORS may be shared with other governmental agencies which may include: Operational Units within DHHS; other departments within the state of Utah; other state IV-D child support agencies; or foreign child support agencies.
Personal data gathered by ORS will be included within the following record series: 30636; 30642; and 30921.
Failure to provide requested personal data may result in the following:
- Delays in establishing and/or enforcing child support obligations;
- Disqualification of benefits such as cash assistance or Medicaid assistance;
- Decisions for child support obligations made by ORS, based upon the best information available; or,
- Closure of your child support case.
